  • "The side effects and appointments are taking over” 
  • “We need a plan for comfort and support” 
  • “We need guidance for what happens next”

What to say to the doctor

 “Can you order a hospice evaluation so we can understand support options at home? We want to focus on comfort and quality of life. If hospice is not appropriate, please tell us what support you recommend instead.”

Optional follow-up line: “We want an evaluation now, not later. Who can we contact today?”

What to say in the hospital

 “We need a hospice evaluation before discharge. Who can place that order today, and when will the hospice team call us?” 

What to say in a facility (SNF/ALF)

“Can you request a hospice evaluation and help us understand which hospices cover this building? We want support for symptom management and caregiver guidance.” 

What happens after you request an evaluation

  • A hospice clinician talks with you about symptoms, daily function, and goals 
  • The hospice reviews medical history and confirms next steps with the physician 
  • You receive a clear plan and you decide whether to start services
